Hospice Eligibility Criteria

  • Serious, Progressive Illness: You must have a serious, progressive illness of any diagnosis.*
  • Focus on Comfort: You have chosen care aimed at comfort and quality of life rather than curative treatments.
  • Assistance with Daily Living: You require help with at least 3 of the 6 Activities of Daily Living (ADLs), which include eating, bathing, dressing, toileting, walking, and managing continence (bladder and bowel control).
  • No Longer Seeking Aggressive Treatment: You have opted to stop aggressive treatments (such as chemotherapy, surgery, or dialysis) that aim to cure or delay the inevitable progression of the illness.
  • Life Expectancy of Six Months or Less: You have been given a prognosis of six months or less to live, as determined by your physician and Haven Hospice’s Medical Director, based on the progression of the illness. Although many hospice patients may outlive these expectations, hospice care ensures the best possible quality of life during this time.
  • Medical Agreement: Both your personal physician and Haven Hospice’s Medical Director must review and confirm that your condition meets hospice eligibility guidelines.

Additionally, patients eligible for hospice often experience frequent hospitalizations or emergency room visits within the past six months due to their illness.

Common Signs & Symptoms of Hospice Eligibility

  • Unmanaged or worsening pain
  • Significant weight loss
  • Difficulty swallowing
  • Persistent shortness of breath
  • Nausea or vomiting that doesn’t respond to treatment
  • Increased fluid retention (edema) or abdominal swelling (ascites)
  • Declining ability to perform daily activities
  • Frequent infections
  • Multiple hospitalizations or ER visits
  • Growing weakness and fatigue
  • Deteriorating skin health
  • Social withdrawal, confusion, or becoming bedridden
  • Progressive decline despite ongoing treatments
